29 CFR 1904.29(b)(1):A Log of all recordable work-related injuries and illnesses (OSHA From 300 or equivalent), was not completed in detail as required by the regulation: a.) Wavepoint 3PL Warehouse - On or about 12/6/24, and at times prior thereto, the OSHA 300 Injury and Illness log was not filled out in the detail required. The injury was not recorded in Calendar Year 2024 on case #4 which occurred on 11/15/24. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Wavepoint 3PL Warehouse - On or about, 12/6/24, and at times prior thereto, the company did not retain the OSHA 300 Injury and Illness log, the annual summary and the OSHA 301 Incident Report forms for Calendar Year 2021.
