The employer did not ensure maintenance employees were provided with training on electrical safety-related work practices such as but not limited to, arc flash hazards, protective equipment for the hands, head and body to protect against electrical hazards while working on or near energized or potentially energized electrical parts. Employees were exposed to 277 volt electrical hazards while troubleshooting light ballasts and replacing bulbs.

The employer did not ensure the electrical energy source was deenergized before the work commenced. The employees working on the light fixture were exposed to electrical hazards of approximately 277 volts.
