29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On or about November 9, 2024, at a worksite located at 210 Crossroads Dr., Plover, Wisconsin; an employee of 7B Building & Development was present at an active jobsite and observed subcontractors being exposed to fall hazards between 10-19 feet and failed to take prompt corrective measures to eliminate the hazard.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  On or about November 9, 2024, at a worksite located at 210 Crossroads Dr., Plover, Wisconsin; subcontractors of 7B Building & Development working from a Genie Aerial Lift (Model: S-40XC, Serial: S40XCH-3219) were exposed to a fall hazard of approximately 10 feet without utilizing the required fall protection system.

29 CFR  1926.760(a)(1):Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.  On or about December 9, 2024, with respect to a worksite located at 210 Crossroads Dr., Plover, Wisconsin: subcontractors of 7B Building & Development were exposed to fall hazards of up to 19 feet while conducting trim work on a new commercial steel erection project.

29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  On or about December 9, 2024, with respect to a worksite located at 210 Crossroads Dr., Plover, Wisconsin: subcontractors of 7B Building & Development using a Louisville 20-foot extension ladder were not protected from falls while access the roofing surface when the ladder was not extended three feet above the landing surface or secured with grab bars installed on the roof.
