On or about December 9, 2024, at the above addressed jobsite, an employee working on the lower roof, under a worker performing residential roofing operations, was not wearing a hard hat to protect against falling materials and debris.

29 CFR 1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   a. On or about December 9, 2024, at the above addressed jobsite, employees using a pneumatic nail gun were not utilizing eye protection, thereby exposing the employees to hazards associated with eye injuries.

On or about December 9, 2024, at the above addressed jobsite, employees performing residential roofing operations, seventeen (17) feet three (3) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b. On or about December 9, 2024, at the above addressed jobsite, employees performing residential roofing operations, eight (8) feet zero (0) inches above the ground,were not protected from falls, thereby exposing employees to fall hazards.
