29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about December 9, 2024, the employer failed to ensure that some form of effective guarding was being utilized on the Lewis Machine Model 11-F Straightening & Cut Machine, to protect employees from hazards such as but not limited to the moving parts, caught in-between, pinch, and/or in going nip points. Employees were exposed to moving parts, including the uncoiling of steel and two sets of rotating wheels, as material is fed through the machine and cut to a specified length.
