29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):   a. On or about November 13, 2024, the employer did not ensure that employees performing maintenance and servicing on the feed throat of the Injection Unit to clear jammed material were protected from machine hazards associated with caught-in/between, contact and struck-by hazards on injection unit 165-5. The employer did not implement energy control application steps as the Injection Unit was not shutdown prior to performing the maintenance and servicing tasks [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, including machine and equipment isolation [(d)(3)], lockout/tagout device application [(d)(4)], dissipation of residual and/or stored energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.  b. On or about December 10, 2024, and at times prior, the employer did not ensure that employees performing setup activities, including tooling/die changes, on the Injection Units, such as unit 55-3, were protected from machine hazards associated with caught-in/between, contact and struck-by hazards on the injection unit. The employer did not implement energy control application steps as the Injection Unit was not shutdown prior to performing the maintenance and servicing tasks [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, including machine and equipment isolation [(d)(3)], lockout/tagout device application [(d)(4)], dissipation of residual and/or stored energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  a. On or about December 10, 2024, and at times prior, the energy control procedures for the Injection Units were deficient in that they lacked the scope, purpose, and specific techniques to be utilized for control of all of the hazardous energy sources. As a result, employees performing maintenance and servicing covered activities were exposed to caught-in/between, contact with, and struck-by hazards.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a. On or about November 13, 2024, and at times prior, the employer did not ensure that each authorized employee performing maintenance and servicing tasks on the Injection Units received adequate training in the methods and means necessary for energy isolation and control. As a result, employees were exposed to caught-in/between, contact with, and struck-by hazards.
