Raynham, MA The employer failed to provide OSHA injury and illness recordkeeping forms 300/300A for the years 2024, 2023, and 2022 until 12/20/2024 after a verbal request was made on 12/10/24, an email request was made on 12/11/2024, and an extension was granted until 12/18/2024 which was requested and agreed upon with the company's safety consultant representation.
