29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about November 27, 2024, employees were exposed to crushing/pinch point hazards and amputation hazard while working on and around the M02 Case Sealer Push Cylinder - Line #2. The employer did not ensure the point of operation was properly guarded when troubleshooting the push cylinder., in-that as the push cylinder is retracted, the edge of the push cylinder against the conveyor wall can cause a pinch point.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: (a) On or about November 27, 2024, the sprocket wheel and chain on the conveyor belt of the M02 Case Sealer - Line #2 front end to the Case Sealer conveyor was not guarded, thereby exposing employees to caught-in/pinch point and amputation hazard.
