The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired. a.) On or about December 10, 2024, the employer failed to ensure that the fixed stairways on the north and south end of the Load Out Dock were maintained in a safe manner as there were approximately 4" openings between the stairway landing platform and the load out dock platform. Employees were thereby exposed to hazards associated with slips, trips, and falls while using the fixed staircase to access the load out dock.

The employees were thereby exposed to caught-in hazards from the unexpected release of energy while repairing paddles inside the conveyor that rotate.
