29 CFR 1904.4(a):The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) At the facility, Complete Care at Laurelton located at 475 Jack Martin Blvd, Brick NJ: On or about 12/10/2024, the employer did not record a work-related illness of a Certified Nurse Aide (CNA) who tested positive for COVID 19 on 8/22/2024 and had lost 10 workdays on the OSHA 300 log or its equivalent. b) At the facility, Complete Care at Laurelton located at 475 Jack Martin Blvd, Brick NJ: On or about 12/10/2024, the employer did not record a work-related illness of a Nursing Supervisor who tested positive for COVID 19 on 8/22/2024 and had lost 10 workdays on the OSHA 300 log or its equivalent. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
