29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift: a) Nazareth Hospital, Front of building, 2601 Holme Ave. Philadelphia, PA 19152 - On or about December 11, 2025, the employer failed to ensure that an employee who was brick pointing while approximately 96 ft high in an aerial lift basket was utilizing a safety harness, exposing the employee to fall hazards
