29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: a) second floor of Maax Bath warehouse; On or about December 9, 2024, employees were removing the plywood flooring on the mezzanine level of a warehouse to replace wooden floor joists with metal floor joists. Employees were working on the edge of the mezzanine without utilizing fall protection measures, exposing employees to falls up to 14 feet.

Employees were not trained in all aspects of fall protection which exposed them to falls up to 14 feet as they worked on the mezzanine level without fall protection.
