29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. On or about December 9, 2024, the employer failed to develop and enforce specific lockout/tagout procedures for the control of potentially hazardous electrical energy powering the rotary valve and its blades inside of Silo FG340 when employees are removing clogged plastic pellets or other non-routine maintenance operations that requires them to bypass the guards and place parts of their body near the rotary valve.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. On or about December 9, 2024, the employer failed to conduct periodic inspections or audits focusing on the specific lockout/tagout procedures of silos including Silo FG340. Silos such as FG340 are powered through electricity and supply energy to items inside of them such as rotary valves and its blades that would need to be stopped for removing clogs or other non-routine maintenance operations.

29 CFR 1904.29(b)(3): The employer failed to enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. On or about December 9, 2024, Advanced Composites, Inc. failed to record an employee's fingers being amputated in their OSHA 300 within 7 days after first learning about the work-related incident. On January 7, 2025, when the employer submitted the OSHA 300 and OSHA 300A logs as part of the inspection there was no entry for the amputation incident.
