Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  BUILDING 25: On December 3, 2024 and at times prior thereto, the data plate on a Hyster Lift Truck, model H60FTM, serial no. L177G04800D, was not changed to reflect the capacity of the truck with the fork extensions at the facility. Use of a forklift with additions such as fork extensions can affect the capacity and stability of the truck. This condition potentially exposed employees to machine tipovers which can cause crushing injuries.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  Building 25: On December 3, 2024 and at times prior thereto, the working space around electrical equipment including a 480 volt panel and three switches was blocked by a piece of plywood and a large fan. Obstructing the electrical panels with large items can increase the amount of time it takes to access the panel in the event of an emergency. This condition potentially exposed employees to an electrical hazard.
