29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) D4 Extruder: On or about December 10, 2024 and times prior, an employee performed maintenance activities such as clearing material from the dryer discharge rotary valve on the D4 Extruder. The employee did not shut off the machine nor lock and tagout the rotary valve.
