29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: On or about December 10, 2024, employees performing work that produced flying particles (using nail guns, hammers and electrical saws) were not guarded from flying debris by eye or face protection.

29 CFR 1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about December 10, 2024, employees performed work from a scaffold more than ten (10) feet above the lower level without the use of a fall protection system during following instances (a-b): a) Scaffold erected on the south side of the home. b) Scaffold erected on the east Side of the home.
