29 CFR 1910.28(b)(3)(iv): The employer did not provide that each employee be protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used. A) In the upper Sorter Area: On or about 12/16/24, the employer exposed employees to a fall hazard in that the gate was not self-closing to two fixed ladders used to access and egress the upper catwalk for the conveyor system. B) In the upper Sorter Area: On or about 12/16/24, the employer exposed employees to a fall hazard in that there was not a self-closing gate or offset to the ladder used to access and egress the upper catwalk for the conveyor system.
