29 CFR 1926.416(a)(1):  The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work:  a.) 231 State Street, Enola, PA, Worksite, House Roof - On or about December 13, 2024, employees were accessing / egressing the roof of the structure to perform residential roofing activities via a Louisville metal extension ladder, and were in proximity to an energized electrical service line feeding into the structure that was not deenergized and grounded, or guarded by insulation or other means.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a.) 231 State Street, Enola, PA, Worksite, House Roof - On or about December 13, 2024, employees who were performing residential roofing work were exposed to a fall hazard of 11-feet and were not utilizing acceptable means or methods of fall protection.   b.) 231 State Street, Enola, PA, Worksite, House Roof - On or about December 13, 2024, employees who were performing residential roofing work were exposed to a fall hazard of 20-feet and were not utilizing acceptable means or methods of fall protection.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
