29 CFR 1910.212(a)(1):One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about December 13, 2024, machine guarding was not properly installed to protect employees from amputation and entanglement hazards when working at and around the Gemini Roll Line (Steven Road) machines. The employer did not ensure that the multiple rollers on top of the energized Steven Road roll line machine was properly guarded near ingoing nip point or pinch point area. Additional, underneath the Steven Road, the roller belt section was not guarded, exposing employees to crushed-by hazards while performing operation. (b) On or about December 13, 2024, machine guarding was not properly installed, bolted, and, or fastened in place to protect employees from amputation and entanglement hazards when working at and around the Gemini Roll Line (51st St) machines. The employer did not ensure that the multiple rollers on the top of the energized 51st St roll line machine was properly guarded near ingoing nip point or pinch point area. (c) On or about December 13, 2024, machine guarding was not properly installed, bolted, and, or fastened in place to protect employees from amputation and entanglement hazards when working at and around the Gemini Roll Line (Craft Master) machines. The employer did not ensure that the multiple rollers on the top of the energized Craft Master roll line machine was properly guarded near ingoing nip point or pinch point area. Additional, the Craft Master was not guarded underneath at the roller belt section, exposing employees to crushed-by hazards while performing operation. (d) On or about December 13, 2024, machine guarding was not properly installed, bolted, and, or fastened in place to protect employees from amputation and entanglement hazards when working at and around the Gemini Roll Line (Konnin) machines. The employer did not ensure that the multiple rollers on the top of the energized Konnin roll line machine was properly guarded near ingoing nip point or pinch point area. Additional, the Konnin was not guarded underneath at the roller belt section, exposing employees to crushed-by hazards while performing operation.
