29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Location - Worksite. On or about December 13, 2025, an employee was not adequately instructed in the recognition and and avoidance of a fall hazard greater than 16' above lower level where fall protection was not used. Employee fell off roof and was transported to hospital.
