29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about December 16, 2024, employees were exposed to struck-by hazards and the employer did not obtain manufacture's prior written approval when a forklift rotator attachment was used to dump raw material.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about December 10, 2024, an employee was exposed to amputation hazards when moving finish product from the spray dry machine because effective machine guarding was not in place in the Rotary Valve to protect the employee from reaching in or allowing the operator's hand to be exposed to rotating parts.
