1910.145(a)(1) These specifications apply to the design, application, and use of signs or symbols (as included in paragraphs (c) through (e) of this section) that indicate and, insofar as possible, define specific hazards that could harm workers or the public, or both, or to property damage. These specifications are intended to cover all safety signs except those designed for streets, highways, and railroads. These specifications do not apply to plant bulletin boards or to safety posters. a) On and before 12/03/24, employees were operating an overhead crane with a coil hook (Coilhaken-type number Z-347013-760-00) to lift steel coils weighing up to approximately 1500 lbs. from the Upender to the Decoiler. Employees were exposed to struck-by and caught-between hazards likely to result in serious injury due to steel coils falling off a hook that was not marked with its rated load, safety labels, and other identifying information such as, but not limited to manufacturer's name and contact, serial number, design category and service class.

29 CFR 1910.179(n)(3)(i): The load shall be well secured and properly balanced in the sling or lifting device before it is lifted more than a few inches. a) On and before 12/03/2024, the employer did not ensure that the steel coil load weighting approximately 1200 lbs., being moved from the Upender to the De-coiler by the overhead crane (5 Ton P&H SN: T46165) utilizing a below the hook attachment, the Coilhaken-type number Z-347013-760-00 coil hook was well secured and properly balanced to prevent the load from falling off the coil hook in that there was a cardboard insert inside the coil's core that was loosely secured via untighten plastic straps which allowed the steel coil to unwind from the center and fall off the coil hook onto the operator at the De-Coiler.
