29 CFR 1903.19(c)(1): Within 10 calendar days after the abatement date, the employer did not certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section:  As of June 11, 2025, the employer had not provided abatement certification for Inspection number 1794062, which was issued on March 6, 2025, and required abatement certification for Citation 2, Item 1, which was due on April 22, 2025, with respect to a work location at 55 Maple Street, Belchertown, MA.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  55 Maple Street, Belchertown, MA: On or about November 19, 2024, three employees were observed working on a residential structure engaged in roofing activities at heights of greater than 6 feet and were not protected by either guard rails, safety net systems or personal fall arrest systems.
