29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: a) Reeb Millwork Corporation of New York, LLC, 7475 Henry Clay Blvd, Liverpool, NY 13088, on or about 12/13/24: Affected employees performed service/maintenance activities including but not limited to, weekly deep cleaning, and did not personally and physically lockout/tagout the equipment (equipment includes, but is not limited to, APEX 990, KVAL 990, and KVAL DL-NCA). To complete the cleaning the employees were required to bypass the guarding on the equipment. The affected employees relied on an authorized employee working in the Maintenance Department to complete the lockout/tagout on the equipment. Abatement certification is required.
