29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). (a) On or about December 18, 2024, the employer had not ensured that employees implemented energy control procedures prior to performing servicing on the Green Press FX Servo 6521 Ermaksan press brake, exposing employees to amputation hazards. The employer had not ensured that energy control application steps were implemented as the equipment was not shut down and turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], verification of isolation [(d)(6)], were not implemented to protect employees from amputation hazards.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. (a) On or about December 18, 2024, the employer had not ensured that employees tasked with performing servicing activities on the Green Press FX Servo 6521 Ermaksan press brake had been trained as authorized employees under the company's hazardous energy control program. Thereby, exposing the employees to amputation hazards. JBT Corp. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1691253, citation number 1, item number 1, group letter a, and was affirmed as a final order on January 17, 2024, with respect to a workplace located at 704 10th Avenue, Grafton, WI 53024.
