29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) Under-construction residential multiplex, northeast side; On or about December 17, 2024, and dates prior, employees who used a JLG brand model 600S telescopic boom lift to access and egress the rooftop at an approximate height of 18.5-feet above ground-level, were not protected from falls by means of a personal fall arrest or restraint system, consisting of a harness and lanyard attached to the anchorage point of the basket.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Under-construction residential multiplex rooftop, various locations; On or about December 17, 2024, and dates prior, employees working from the rooftop to install structural sheathing were not protected from falls while working at approximate heights ranging between 18-feet, 4-inches, to 24-feet above ground-level. (b) Under-construction residential multiplex, second story; On or about December 17, 2024, and dates prior, employees performing tasks associated with framing construction were not protected from falls while working near unguarded window openings with low sill heights and with bottom edges up to 12-feet, 9-inches above exterior ground-level.
