29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) On December 18, 2024, at the above addressed jobsite, employees performing roofing work on residential home, approximately 9 feet above the ground below on the north side of the home, were not secured to a personal fall arrest systems or other means of fall protection, thereby exposing the employees to fall hazards.    b) On December 10, 2024, at the above addressed jobsite, employees performing roofing work on residential home, approximately 22 feet above the ground below on the south side of the home, were not secured to a personal fall arrest systems or other means of fall protection, thereby exposing the employees to fall hazards.    No abatement certification or documentation is required for this item.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On December 18, 2024, at the above addressed jobsite, the two portable ladders used by employees to access and egress the roof over on the north side, measuring approximately 9 feet from ground to the eve, were not extended to have the side rails extend at least three (3) feet above the upper landing.    No abatement certification or documentation is required for this item.

29 CFR  1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately.  a) On December 18, 2024, at the above addressed jobsite, the portable ladder near the northwest corner of the roof was used by employees to access and egress the roof over on the north side, measuring approximately 9 feet from ground to the eve, with defects to the side rail and foot on the west most rail.  No abatement certification or documentation is required for this item.
