29 CFR 1910.305(g)(1)(iii):  If used as permitted in paragraphs (g)(1)(ii)(C), (g)(1)(ii)(G), or (g)(1)(ii)(I) of this section, the flexible cord shall be equipped with an attachment plug and shall be energized from an approved receptacle outlet.  On or about December 3, 2024, at an establishment located at 170 Evergreen Road, Oconto, Wisconsin; in the Siemen Panel Project area, the #1 ToughFlex Heavy Duty Welding Cable did not have attachment plugs at the end of the cable lugs potentially exposing employees to electrical shock hazard when the Siemens electrical disconnect was in the ON position.

The employer shall maintain a written copy of the procedures outlined in paragraph (b)(2) and shall make it available for inspection by employees and by the Assistant Secretary of Labor and his or her authorized representatives. Note: The written procedures may be in the form of a copy of paragraph (b) of this section.  On or about December 3, 2024, at an establishment located at 170 Evergreen Road, Oconto, Wisconsin; in the Siemen Panel Project area, testing instructions of the Passenger Coach Charging System Type:  PCCS-68S did not have specific procedural steps for the placement and removal of the 480 VAC power supply cables exposing employees to electrical shock hazards.

Qualified persons (i.e. those permitted to work on or near exposed energized parts) shall, at a minimum, be trained in and familiar with 29 CFR 1910.332(b)(3)(i) to 1910.332(b)(3)(iii):  On or about December 3, 2024, at an establishment located at 170 Evergreen Road, Oconto, Wisconsin; in the Siemen Panel Project area, the employer did not provide training to qualified persons discussing the safety-related work practices as well as appropriate personal protective equipment required when performing testing on the passenger coach charging system exposing employees to electrical shock hazards.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed.  Note: Personal protective equipment requirements are contained in subpart I of this part.   On or about December 3, 2024, at an establishment located at 170 Evergreen Road, Oconto, Wisconsin; in the Siemen Panel Project area, while performing electrical testing activities on the passenger coach charging system, the employee did not use electrically rated gloves exposing employees to electrical shock and thermal burn hazards.

29 CFR 1910.147(c)(5)(ii):  Lockout devices and tagout devices shall be singularly identified; shall be the only devices(s) used for controlling energy; shall not be used for other purposes; and shall meet the following requirements:  On or about December 18, 2024, at an establishment located at 170 Evergreen Road, Oconto, Wisconsin; in the Siemen Panel Project area, the 480V AC electrical disconnect utilized a lockout device for purposes other than under lockout.  The lock and tag were used to prevent unauthorized employees from turning on the 480V breaker.

29 CFR 1910.1200(h)(1):  Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  On or about December 13, 2024, at an establishment located at 170 Evergreen Road, Oconto, Wisconsin; in the Siemen Panel Project area, employees working around sealed lithium-ion batteries during the assembly of the Passenger Coach Charging System were not trained on the categories of hazards.
