On or about December 19, 2024, in the thermite cell during the start charge module operation of the SCEPS program, employees were exposed to flying particles, molten metal and fire hazards in that, the employer did not enforce the use of eye and face protection during the start charge module installation.  b. On or about December 19, 2024, in the welding cell: employees were exposed to radiant energy hazards in that, the employer did not enforce the use of eye protection while conducting contact welding.
