29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  Location: Job site;  On or about 12/19/2024, an employee was observed working in a hazardous area beneath an active siding project without a hardhat. Improperly secured equipment, including a crawling board (chicken ladder), siding materials, and other objects, posed a risk of falling and striking the employee.

29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) was not protected by a personal fall arrest system, a guardrail system, or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board.  Location: Job site;  On or about 12/19/2024, the company owner and job site foreman were working from a crawling board (chicken ladder) 15-feet or more above the ground without the use of personal fall arrest systems, guardrail systems, or by a grabline or equivalent handhold.

Location: Job site;  On or about 12/19/2024, employees were allowed to utilize a crawling board, 15-feet or more above the ground below, which was not secured to the roof by a ridge hook or by other means.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  Location: Job site;  On or about 12/19/2024, employees were allowed to engage in siding activities without the use of fall protection from a residential roof that was 15-feet or more above the ground below.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were used at an angle such that the horizontal distance from the top support to the foot of the ladder was not approximately one-quarter of the working length of the ladder.  Location: Job site;  On or about 12/19/2024, the company owner and an employee utilized a Werner portable extension ladder which was erected at approximately a 59-degree angle and exceeded the 4:1 ratio (75-degree angle) for safe use.
