29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s). Location: Job site; On or about 12/19/2024, Riera Construction Inc., as a controlling employer did not ensure that frequent and regular inspections of the job site were performed by a competent person. As a result, subcontractor employees from EBR Construction Corp were exposed to hazards in the following instances: Instance a: Fall hazards where no fall protection was provided. Instance b: Struck-by hazards where no hard hats or head protection was provided. Instance c: Fall and collapse hazards where ladders were erected beyond a 4:1 ratio.
