29 CFR 1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part. a. On or about December 19, 2024, at 3691 Kassidy Drive, Lot 82, Grove City, Ohio, 43123, the employer failed to develop and implement a safety and health program for employees engaged in installing roofing shingles on a two-story, single-family home. As a result, the employees were exposed to fall hazards of up to approximately 22 feet to the ground below. Additionally, employees were exposed to struck-by hazards to the face and eyes while operating a pneumatic nail gun at distances of approximately 12 to 24 inches.

On or about December 19, 2024, at 3691 Kassidy Drive, Lot 82, Grove City, Ohio, 43123, the employer failed to ensure the employees were wearing eye protection while operating pneumatic nail guns while installing roofing shingles on a new build two-story single-family home. The condition exposed the employees to struck by hazards to the face and eyes.

The lack of fall protection exposed employees to falls of up to approximately 22 feet to the ground below.
