29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:  On or about December 18, 2024, an employee was exposed to fall hazards above ten feet while being raised by a Genie GTH-1056 Rough Terrain Forklift equipped with a personnel lifting platform and the employer had not ensured the use of an adequate fall protection system.

29 CFR  1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:   On or about December 18, 2024, an employee was exposed to fall hazards when occupying a platform raised by a Genie Model GTH-1056 Rough Terrain Forklift and the employer had not evaluated the Genie GTH-1056 operator at least every three years in topics such as, but not limited to, controls of the truck.
