The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used. a) On the roof of the carports located at 14561 Legends Blvd N, FORT MYERS, FL 33912 On or about 12/19/2024, the employer exposed employees to a fall hazards, in that, fall protection equipment was not installed or utilized while employees were performing tile roof installation and other roofing activities at an approximate 9 feet 6 inches in height above ground.
