The side rails of the portable extension ladder did not extend at least 3 feet above the landing surface of the roof, and the ladder was not secured.  The side rails extended approximately 18 inches.  Pizano Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1755398, citation number 1, item number 3 and was affirmed as a final order on 11/05/2024, with respect to a workplace located at 23605 51st Terrace, Shawnee, Kansas.
