29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) Ground level and First Story: On or about 12/20/2024, the employer exposed employees to struck-by hazards while operating a Komatsu PC 360 LC to lift trusses, in that protective helmets were not provided to employees exposed to head injuries from falling objects.

29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:   a) Ground level: On or about 12/20/2024, the employer exposed employees to struck-by hazards while operating a Makita circular saw to cut lumber in that, eye protective equipment was not provided to employees using the cutting saw.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Leading edge: On or about 12/20/2024, the employer exposed employees to fall hazards of approximately 13-feet to the lower ground level, in that a fall protection system was not provided to employees engaged in putting up trusses on the roof.
