29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. On or about December 26th, 2024, employees engaged in concrete operations located at 250 West Vine Street, Fremont, Nebraska 68025, were exposed to fall hazards in that there were two holes on the walking working surface located on each side of the main hopper, on the 2nd floor of the plant. i. On the right pouring side of the hopper located on the 2nd floor of the plant maintenance/operators were exposed to a fall through hazard in that the employer failed to guard a 40x6 hole. ii. On the left pouring side of the hopper located on the 2nd floor of the plant maintenance/operators were exposed to a fall through hazard in that the employer failed to guard a 36x10 hole.

29 CFR 1910.305(g)(2)(iii):Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about December 26th, 2024, employees engaged in concrete operations located at 250 West Vine Street, Fremont, Nebraska 68025, were exposed to fire hazards in that a 480V, 3 phase conductors, ran to the motor that provides power to the main hopper on the 2nd floor of the plant was not provided with strain relief. The outer jacket of the conductors was separated from the box that the conductors was entering.
