Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. On or about December 26, 2024, at the facility located at 306 Park Street, Spencer, Wisconsin; the employer did not ensure the Crown standup forklift (Model No.: RC5715-30, Serial No.: 10507275) was equipped with a tag or decal for using the Cascade barrel dumper attachment (Catalog No.: 15D-FDD-69564, Serial No.: 2756456-T4).
