29 CFR 1917.43(c)(5): Powered industrial trucks were not maintained in safe working order. a) On or about December 26, 2024, and at times prior to, at 2650 Port Boulevard, Miami, FL 33132, located at the Genset Loading/offloading station employees were exposed to a struck by hazard while operating a Caterpillar forklift (Model No. PD8000, Serial No. AT12B00525) that did not have an operable back alarm. b) On or about December 26, 2024, and at times prior to, at 2650 Port Boulevard, Miami, FL 33132, located at the Genset Loading/offloading station employees were exposed to a struck by hazard while operating a Caterpillar forklift (Model No. PD8000, Serial No. AT12B00525) that was missing a bolt on the rear rim wheel.

On or about December 26, 2024, and at times prior to, at 2650 Port Boulevard, Miami, FL 33132, located at Genset Loading/offloading station - a Caterpillar forklift (Model No. PD8000, Serial No. AT12B00525) using a Reefer Service Platform where platform did not have updated capacity plates to conform with platform attachment.
