29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:  (a) Franklin Court Apartments, Superior, WI; On or about December 27, 2024, occupants of a trailer mounted JLG brand boom lift did not keep their feet firmly positioned on the platform floor while it was elevated approximately 10 feet, 2 inches above the ground, in that, one of the employees working from the elevated platform climbed the railing and stood on the mid-rail, to remove upper bolts that secured the awning frame to the building.

29 CFR 1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e):  (a) Franklin Court Apartments, Superior, WI; On or about December 27, 2024; Fall restraint or personal fall arrest systems were not worn while two employees removed components of an awning from the platform of trailer mounted JLG brand boom lift at heights of approximately 10 feet, 2 inches above the ground.

29 CFR 1904.29(b)(1): The log of occupational injuries and illnesses, (OSHA Form No. 300), was not completed in the detail provided in the form and the instructions contained therein:  Advanced Design Awnings & Signs, Inc -  1600 29th Street, Cloquet, MN.  On or about December 27, 2024, the employer did not identify the case number (column A), provide a description to identify the body part and extent of injury (column F), and/or classify the case type (columns G-J) on the OSHA Form 300 or equivalent for calendar year 2024.  (a) On or about October 9, 2024, an employee working in the shop tripped on a trailer hitch, which resulted in 1 day away from work.  This was the 6th injury written on their log, but the case number was not written in column A.  (b) On or about October 9, 2024, an employee working in the shop tripped on a trailer hitch, which resulted in 1 day away from work.  The body part injured and type of injury was not described in column F.  (c) On or about October 9, 2024, an employee working in the shop tripped on a trailer hitch, which resulted in 1 day away from work.  The injury type was not classified by marking one of the categories in columns G-J.
