29 CFR  1910.22(a)(2): The employer did not ensure that the  floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided.   a) In the utility room, on or about 12/27/2024, employees are exposed to trip and fall hazards, in that, the water drain had an uncovered floor opening measuring 1'2"x 7".

29 CFR  1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) In the utility room, on or about 12/27/2024, employees were exposed to chemical burns when dispensing NIL-8, a sanitizing solution categorized as corrosive, into a 500-gallon tank for water sanitization, in that, the employer did not provide appropriate eye and face protection against chemical splash.

29 CFR  1910.303(g)(1)(i): Work space was less than 30 inches wide in front of electric equipment operating at 600 volts, nominal, or less:  a) In the utility room, on or about 12/27/2025, employees were exposed to electrical hazards, in that, a 120/208-volt electrical panel access was obstructed by a pallet of salt bags.
