Street, Delray Beach, FL 33434, an employee while repairing the attic door was exposed to approximate 11.5 ft. fall hazard; in that, the employer did not ensure that a personal fall protection system such as personal fall arrest, travel restraint, or positioning system was in use to prevent falls from an  elevation onto the concrete floor.
