29 CFR  1910.1026(h)(1): A hazard was present or was likely to be present from skin or eye contact with chromium (VI); however, the employer did not provide appropriate personal protective clothing and equipment at no cost to employees, and/or did not ensure that employees used such clothing and equipment:  a) On March 4, 2025, En-Chro Plating, LLC did not ensure that employees used the appropriate personal protective equipment including safety glasses, long sleeves and/or arm covers and gloves when lowering bars into Hydraulic Bar Tanks #9 and #13, which contained chromic acid.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1904.41(a)(1)(i): The establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and the establishment was classified in an industry listed in appendix A to subpart E of this part, and the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee by the specified date:   a) On or about March 2, 2025, En-Chro Plating, LLC did not electronically submit information from its OSHA Form 300A Summary of Work-Related Injuries and Illnesses for calendar year 2024 to the Injury Tracking Application. The establishment employed 35 employees and was classified under NAICS 332813 during calendar year 2024.  No abatement certification or documentation is required for this item.

29 CFR 1910.1026(k)(5)(i): The employer did not obtain a written medical opinion from the physician or other licensed health care professional within 30 days for each medical examination for chromium (VI) performed on each affected employee:  a) On or about March 4, 2025, En-Chro Plating, LLC did not obtain a written medical opinion from the physician or other licensed health care professional within 30 days for each medical examination for chromium (VI) performed on employees exposed to chromic acid at or above the action level when working the Hydraulic Line.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  a) On December 30, 2024, En-Chro Plating, LLC did not ensure that each tank containing chromic acid located in the Hydraulic Bar and Upper Deck was properly labeled. Tanks #2 and #6 in the Upper Deck and tanks #7, #9, #10, #11 and #13 in the Hydraulic Bar were not labelled.   b) On December 30, 2024, En-Chro Plating, LLC did not ensure that each secondary container of WD-40, which contained petroleum base oil and aliphatic hydrocarbon, was properly labeled. A 24-oz spray bottle located at the Polisher #2 Station and the Upper Deck were not labelled.  No abatement certification or documentation is required for this item.
