ABATEMENT DOCUMENTATION REQUIRED

29 CFR  1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices:  a) On or about 12/30/2025, at ADM Milling, 250 Ganson St, Buffalo, NY; where unrestrained potential energy from the raised weight of a scissors lift caused the scissors lift to drop, creating a pinch point which resulted in an amputation injury.  ABATEMENT DOCUMENTATION REQUIRED
