29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a. At 3011 International Golf Parkway, Saint Augustine Florida, On or about December 27, 2025, the employer exposed two employees in the 10-foot deep excavation to cave-in hazards in that the open ends of the trench box where employees worked were not protected from cave-in of the trench sidewall soil.
