29 CFR 1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): Lerch RV, 70 Commerce Drive, Milroy, PA: On or about January 2, 2025, employees were exposed to fall hazard of at least 10 feet to the floor while working on the roofs of recreational vehicles without an adequate fall protection system being utilized. No abatement certification or documentation required.

29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent. a. 70 Commerce Drive, Milroy PA - The employer failed to implement and retain OSHA recordkeeping forms, the employer did not have an OSHA 300 or equivalent or any record of OSHA 300A forms for previous calendar years of operation. The employer had not developed and implemented a recordkeeping program to document and retain OSHA forms or equivalents for the requisite 5 years. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
