29 CFR 1915.74(c)(2): A ramp meeting the requirements of 29 CFR 1915.74(c)(1) was not provided where employees had to step from a wharf, float, barge, or river towboat, and employees could not step safely on to the vessel.  Pamala Ann Barge and Derrick Barge Pacific: On January 01, 2025, and at times prior thereto, a suitable gangway or ramp meeting the requirements of 1915.74(c)(1) was not provided where employee(s) could step safely from the Pamala Ann Barge to the Derrick Barge Pacific. The spacing between barges was measured at approximately 3.8 feet at its widest point. This condition exposed employee(s) to falls and drowning hazard(s).

29 CFR  1915.81(b)(1)(iii): The employer did not ensure that each walkway was kept clear of tools, equipment, materials, and other objects that created a hazard for employees:  Pamala Ann Barge: On January 01, 2025, and at times prior thereto, walkways were not clear of pipes, air hoses, electrical conduit, metal chain, and other material which created a hazard for employee(s) when walking on the barge. This condition exposed employee(s) to slip-trip and fall hazard(s).

29 CFR 1915.152(b)(4): The employer failed to document an occupational hazard assessment which should have included: Occupations involved in the survey, the date in which the survey took place and the name of the person who performed the hazard assessment.  Pacific Maritime Group Inc: On January 01, 2025, and at times prior thereto, employee(s) would enter and exit vessels such as barges and tug boats to conduct assigned tasks. The employer did not ensure to document a Job Safety Analysis which included occupations involved, the date in which the survey took place, and the name of the person who performed the survey. Failure to document and communicate the required hazard assessment to effected employees potentially puts them at risk for occupational injury.
