An employee was injured while operating a CMZ-7R Power Rotary Machine that was not guarded.

29 CFR 1910.147(c)(4)(ii) The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance: a) On or about January 3, 2025, and at times prior, machine specific procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were performing service or maintenance on machinery in the facility, including, but not limited to a CMZ-7R Power Rotary Machine. The Lock Out/Tag Out procedure did not list machine specific information including: a specific statement of the intended use of the procedure; specific procedural steps for shutting down, isolating, blocking and securing the machine or equipment to control hazardous energy; specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints and/or terminal screws. a) On or about January 3, 2025, and at times prior, the employer, did not ensure employees were safe from electrical hazards in that a foot pedal switch used to operate the CMZ-7R Power Rotary Machine had exposed wires between the conduit and the foot pedal.
