As a result, on January 3, 2025, workers were exposed to fall hazards of up to 10 feet to to the ground while conducting roofing activities where fall protection was not used.

1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways: Location: 123 Milk St, Westborough, MA On January 3, 2025, subcontractor employees were exposed to falls from heights up to 10 feet to the ground while accessing an upper landing surface where an extension ladder was used to access a residential roof, the ladder was set up on the gable end of the roof, and the side rails of the ladder were not supported equally. The employer did not ensure that their employees were adequately trained.
