29 CFR 1910.106(e)(6)(i): "General." Adequate precautions were not taken by the employer to prevent the ignition of flammable vapors. Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat. On or about January 3, 2025, employees working in close proximity to the wave solder tin line were exposed to fire hazards as the employer was storing an alcohol based chemical (Alpha EF-8300LR Flux) near a melting pot which can serve as an ignition source. The wave solder tin line contains a melting pot that heats up to 250 degrees Celsius. The flux has a flashpoint of 11.667 degrees Celsius.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided by the employer within the work area for immediate emergency use. On or about January 3, 2025, employees are required to use ALPHA EF-8300LR FLUX, ORGANO FLUX 3355-HB, and ALPHA NEUTRAL FLUX 373 on the soldering lines. The chemicals contain acids and the ALPHA NEUTRAL FLUX 373 is a corrosive. The employer did not ensure that employees were provided with a suitable facility capable of flushing the eyes for at least 15 continuous minutes. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1910.1025(h)(1): The employer did not ensure that all surfaces were maintained as free as practicable of accumulations of lead. On or about January 3, 2025, employees working on the wave solder tin line (lead soldering) machine, were exposed to an accumulation of lead directly above the door handle on the machine and above the product conveyor exit. Wipe samples taken for lead on January 22, 2025, showed 36.604 micrograms per 10cm2 on wipe 2 and 11.722 micrograms per 10cm2 on wipe 3. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following: On or about January 3, 2025, employees working with hazardous substances such as lead or various forms of FLUX, were not provided with a written hazard communication program. Employees use FLUX on the soldering lines before solder is applied in order to clean and prepare the boards for soldering. The wave solder tin line uses lead as a soldering paste for certain customers. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1910.1025(d)(2): Initial determination. The employer who has a workplace or work operation covered by this standard did not determine if any employee may be exposed to lead at or above the action level. On or about January 3, 2025, employees working on the wave solder tin line, were not provided with initial monitoring to determine what their baseline lead levels were. Employees melt lead to turn it into a paste to solder electronic boards. This item was corrected during inspection.
